About FormJuicer
FormJuicer is a UB-04 claim audit tool and billing reference. Most institutional claim denials are mechanical — a type of bill whose frequency digit does not exist, an emergency-room line with no procedure code, an attending NPI that fails its check digit. None of that is medicine. It is data quality, and it is checkable before a claim is submitted.
This site is two things: a browser-based audit engine that runs payer-style edits against a claim, and a field-by-field reference for the UB-04 (CMS-1450) form.
Our Product
The audit engine runs entirely client-side. Load the sample case and it scores the claim, explains every flag in plain language, and suggests the fix:
- Validate the claim against rule families payers apply — type of bill, revenue code ↔ HCPCS pairing, NPI check digits, date logic, required-field presence
- Explain each flag with a plain-English reason and a suggested correction
- Score the claim so you can see defects clear as values are corrected
Current status: the engine runs on de-identified sample data. Local parsing of your own PDF or pasted claim text is not built yet. We would rather say that plainly than imply a self-serve upload that does not exist.
Why a Knowledge Base?
Auditing a claim only helps if you know what each field is supposed to contain. The reference covers all 81 UB-04 form locators, plus the code sets that hang off them — revenue codes, condition codes, occurrence codes, value codes, type of bill, and patient discharge status.
We built it because:
- Field-level questions are the common case — "what does revenue code 0450 mean?", "which digit of the TOB is the frequency?"
- Building audit rules requires understanding every field — you cannot check a code pairing you cannot define
- A good audit starts with good definitions — an unambiguous field definition is what makes a rule explainable
Who We Serve
- Medical billers who need quick field lookups and denial resolution steps
- Coders (CPC, CCS, RHIA) who need accurate code references and modifier guidance
- Revenue cycle managers looking for compliance assurance and process improvement
- Personal-injury, lien, and bill-review teams who receive institutional bills they did not generate
- Students and new hires entering medical billing who need structured reference material
Our Content Approach
Every article follows one principle: if you read it, you should be able to act on it. We write procedural, specific, verifiable content rather than abstract overviews.
- Specific codes and field numbers — not "fill in the appropriate revenue code" but "enter revenue code 0450 for emergency room services"
- Step-by-step resolution — not "contact the payer" but "call the payer's provider services line on the remit, reference the claim number, and request a reopening citing the corrected diagnosis code"
- Cited sources — factual claims link to official publications, regulatory documents, or authoritative references
Editorial Standards
- Source verification — code references, field specifications, and compliance rules are traced to primary sources (CMS, NUBC, MAC publications, state Medicaid guides)
- Fact-checking — claims are cross-referenced against authoritative references before publication
- Regular review — content is reviewed on a scheduled basis as code sets and form specifications change
- Corrections policy — errors are corrected promptly and transparently (see our Corrections Policy)
Content Scope
Coverage is limited to what we actually maintain. We do not list form types we do not support.
| Category | Coverage |
|---|---|
| UB-04 Form | All 81 form locators with box-by-box instructions |
| Type of Bill | Facility, care, and frequency digit breakdowns |
| Revenue Codes | Categorized by service type |
| Condition / Occurrence / Value Codes | Code definitions with usage guidance |
| Denial Codes | CO, PR, OA reason codes with resolution steps |
| CPT/HCPCS | Common codes with modifier guidance |
CMS-1500 is covered only where it differs from the UB-04 — we do not audit or process professional claims.
